Score every rule, not only the soft ones

TT-16 scored the soft rules only, on the reasoning that hard violations decide publishability and should not move a number about fit.

That leaves a gap. A plan can be unpublishable in one small way or in twenty and the score reads the same for both, so the only thing telling them apart is a list somebody has to count. How far from publishable a plan is turns out to be a real question, and the score cannot answer it.

KB-8 has the arithmetic and the reasoning. This implements it.

Acceptance criteria

• Every rule on KB-2 contributes to the score, hard and soft alike, on the same opportunities-and-misses basis TT-16 already uses
• A hard rule's weight defaults to 3 and a soft rule's to 1. Each may still override its own, in its own file, without an edit to anything shared
• A rule the guest list gave no chances is left out of the mean rather than counted as perfect
• The score never decides publishability. A plan publishes when its hard violation count is zero and for no other reason, whatever it scores
• The score is never shown on its own. Wherever it appears, whether the plan can go out appears with it, in words
• The per-dimension breakdown lists hard rules before soft ones, matching the violations panel
• Adding a rule later changes the score with no edit to the scoring code, as now

Notes

Every plan's figure moves the day this lands, and it will move down for most of them, because plans that scored on soft rules alone now carry their hard misses too. That is the point rather than a regression, but it is worth saying out loud to anybody who has started recognising the number.
